Privacy Statement

On 21st December 2001 new privacy laws came into operation. At Barossa Village, your right to Privacy is important to us.
This document explains the types of personal information that we keep on record and how we may use that information. It also provides some detail about your privacy rights along with our rights and obligations.
All application forms will include notices about privacy and requests for consent. Each application form will specify how any information about you may be used and disclosed, in relation to the service provided.

Collection

Our records include information that identifies you (like your name and address) and other information supplied by you and/or people nominated by you. During the course of your admission we may also hold or collect information from other sources such as health providers. This information is used to help us form views about how we can improve your care.

Use of Disclosure

We use your personal information to:
        1. help us provide a service to you
        2. efficiently manage and administer all services
        3. ensure our internal operations are running smoothly, which
                includes all legal requirements

Disclosure

Depending on the service we are providing to you, we may disclose your personal information to:

Our external providers, who provide a service to us.  This is on a confidential basis and may include, for example, organisations that provide pathology or radiology services.

Anyone acting on your behalf such as your financial advisor or solicitor - unless you tell us not to.

Additionally, we will disclose information if we are required by law to do so, or if you provide consent for us to do so

Your personal information may also be used by us to inform you about certain activities, or services provided by us, which we consider might be of interest to you.

By accepting the service, you consent to Barossa Village maintaining, using and disclosing your personal information as set out in this document.

Your Rights

You can request access to the personal information that we keep on record. If you believe that any information is incorrect or out of date you may ask us to correct it. There is no fee for requesting access to your information; however, we may charge you the reasonable cost for processing your request.  At any time, you may change your decision regarding who your information may be disclosed to.

Advocacy Information

Negotiating through the Aged Care System can be stressful, especially through a time when an older person's health may be compromised.

An advocate is a person or organisation who can help you to uphold your rights.  In most situations we are able to advocate for ourselves.  However changes which occur in our lives can make one feel in a vulnerable position.  If you are in a situation where you are unable to express your wishes, an advocate can speak up for you.
