The TCP is a short term restorative care program for older persons who have been in hospital and are expected to benefit from rehabilitation & support to assist them to get to a lower level of care and to shorten the hospital stay.  

The service (TCP) can be provided either in your own home or in a residential facility or a combination of both.

During your time on the program, you will participate in goal setting, enabling you to be supported and encouraged through planned services to gain your best possible level of function.  

Fees: Clients will usually be required to pay the aged care daily care fee to access the programme.
